This booklet is your space to pause, reflect, and refocus.Use it to look back at what’s worked, learn from what hasn’t, and set goals that keep you moving forward — one small habit at a time.There’s no right or wrong way to fill it in — just be honest, specific, and realistic. Take your time. Think deeply. Write clearly.

REFLECTION
Before setting new goals, take stock of where you are.

  • What have you achieved since January?
  • Celebrate progress — big or small.
  • What challenges have you faced?
  • Recognise what’s been tough and what you’ve learned.
  • What habits or attitudes held you back?
  • Be honest — awareness creates change.

Tip: The clearer you are here, the more focused your next steps will be.

AUTUMN MINDSET
This section is about how you think and respond when things get hard.

  • What could you do differently this time?
  • Learn from moments that threw you off before.
  • What mindset will help you stay consistent?
  • Choose words like “patient,” “disciplined,” or “resilient.”
  • Who or what supports you when motivation dips?
  • Plan ahead — know your support network and your “why.”

Autumn isn’t about perfection — it’s about steady, sustainable action.

GOAL SETTING
Now, look ahead to the rest of the year.

Why does this goal matter?

  • Connect it to your bigger purpose — how will it improve your life?
  • What do you want to achieve or maintain by year-end?
  • Keep it simple: 1–3 goals that are clear and achievable.
  • What might get in the way?
  • Identify challenges — then write one way to stay on track.
  • What small habits will move you forward?
  • Focus on daily actions you can control.

Goals without habits fade — habits make goals happen.
